Hi,

That would indicate that the a pin reset and a soft reset has been performed. Do you perform any of this through the debugger? Are you using a DK or a custom board? Is the Reset pin routed out?

Can you read the register directly by using nRF Command line Tools like this:

nrfjprog --memread 0x40000400
